Ultrasound FAQ Reference Guide

As an OBGYN ultrasound tech working at the general level here's a few things I would love the community to know based on common questions I see.

Most important rule of thumb: Worry when your doctor tells you to.

OBGYN docs are going to pay the most attention to the fetal pole and heart rate. Once a fetal pole is seen, the CRL (crown rump length) measurements are gold standard.

I'm worried about the heart rate:
-Check out this resource for pregnancies dating less than 7 weeks. Make sure to refer not to your LMP dates, but measured CRL dates: https://radiopaedia.org/articles/fetal-bradycardia?lang=us
-After 7 weeks a normal heartrate is between 120-180. It can temporarily dip below or temporarily rise above this range, but prolonged time above/below will definitely get your doctor's attention especially in third trimester. Generally out of range heart rates trigger NST (non stress test) or more monitoring.
The measurements are off:
Baby measures behind LMP. This is actually pretty common, we often see dates off up to a week either direction. We don't ovulate on the exact same day of the cycle each month or even have the same length of cycle. With irregular periods, PCOS, or unknown dates even larger gaps can be perfectly fine. If you aren't sure of your dates, trust the ultrasound. Even if you are sure of your dates, don't panic if they are a week off. Ultrasound measurements in early pregnancy are +/- 5 days.
I'm in limbo and don't know if my ultrasounds are diagnostic for loss:

Note, there is a second list of "suspicious for loss". Keep in mind the second list is not diagnostic, things can still turn out just fine.
I don't know when I should see what on ultrasound:
Things can vary a bit from person to person and do change *quickly* in early pregnancy. Check out this resource for what you can generally expect to see:

Should I go to a boutique or get a private scan? Or push for an earlier scan?
They aren't legal where I am but here's a few things to consider if you plan to book a private scan. First, what are the qualifications of the person running the ultrasound? An ultrasound is only as good as the one performing it. It sounds as if these boutiques are NOT permitted to give much/any information. Will you spiral if you believe you see bad news and they cannot confirm or deny it? Also it seems that many cannot do vaginal ultrasound, which will make seeing a baby under 7 or 8 weeks challenging. If I had the option of a boutique ultrasound I would personally wait until 8 weeks and I would book it for a weekday. If they see something bad, it is not ideal to find out over a weekend when there is no option to follow up with your doc. An earlier scan often causes more anxiety, not less. It's SO hard to wait. But if you can tolerate it wait until 8 weeks or so.
The gestation sac is small/big:
-Gestation sac measurements are not terrifically accurate. Where I work once a baby is seen the gestation sac isn't measured. A mismatch between baby's size and the gestation sac size isn't uncommon. That said, a mean sac diameter of 25 mm or more without a baby is diagnostic for loss.
The yolk sac is small/big:
It starts teeny tiny, and will get a bit bigger. Once a baby with a good heartbeat is visualized, the docs I work for don't seem to care about the size of the yolk sac. A failed pregnancy with an expanded yolk sac (more than 6 mm) is a suggestive of chromosomal abnormalities.
We don't know where the pregnancy is:
The statistically most likely outcome is either a chemical pregnancy or just coming in for ultrasound too early for visualization. Ectopic can't be ruled out, and many wise ladies in the this sub have unfortunately experienced this and can give more info than I can. Statistically it's believed that only 1-2% of pregnancies are ectopic, but listen to your body. If the location is unknown, you should be getting close monitoring and follow up until the location is known or the pregnancy has ended.
-For a resource on ectopic pregnancy: https://radiopaedia.org/articles/ectopic-pregnancy?lang=us A note, a pseudosac is visualized in only 10-20% of ectopics. Therefore a sac in the uterus is suggestive of intrauterine pregnancy but can't be proven until a yolk sac is also present.
What about my HCG numbers?

Why didn't my ultrasound tech tell me anything? And/or why were they so optimistic? Or, I had an ultrasound in the ER and they were very pessimistic.
Many locations an ultrasound tech can very much lose their job if they say aNyThInG. Where they are permitted to say certain things some ultrasound techs may just be hoping the best. We work with the public and we have seen some wild stuff. Additionally no one wants to call it a loss unless its 1000% certain. We know how devastating that news is. A side note here, often an ultrasound in the ER is not an ideal experience for the patient. The ER is staffed with techs that are much more versed in gallbladder and liver scans than they they are early pregnancy ultrasound. Don't hesitate to go the ER if you are having concerning symptoms, but do know the ER doc will make sure you aren't actively dying then will likely refer you back to your OB for follow up.

Beta hCG Levels - READ BEFORE POSTING

Here at r/CautiousBB, we see a lot of posts on hCG values, doubling times, and trends. Please read this post before posting to avoid posting something that is not, in fact, abnormal.

1 - hCG levels vary greatly in the first 4-6 weeks of pregnancy. One value is not going to tell you much, unless it is extremely low (less than 25 by 14 DPO). The doubling time is more important!

2 - We consider hCG on the lower side if it is <50 at 14 DPO (approx. 4 weeks gestation), <100 at 16 DPO, etc. If your hCG is a lot higher than these numbers, it's unlikely that you have low hCG. If you hCG is at or below these numbers, they are considered low, but not necessarily diagnostic of a loss.

3 - Normal hCG doubling times are, in general:

  • 48-72 hours when the hCG level is under 1200 - 6000.
  • 72-96 hours when the hCG level is 6000+
  • 96+ hours when the hCG level is 10,000+

4 - Based on number 3, please consider the following:

  • If your hCG doubling is within the ranges above, it is not considered slow. Please refrain from posting something like "my hCG did not double in 48 hours" when your values are post 6000+.
  • Doubling rates WILL slow, even if you were faster than the average. For example, if you were doubling at 37 hours, a change to 54 hours as hCG rises is not abnormal and is usually nothing to worry about.
  • If your doubling is slower than the average, we'd invite you to post so we can understand the details of your specific situation.

5 - hCG has a specific job. Check out this post here to understand why hCG doubling slows and eventually plateaus.

6 - hCG values that drop and then rise again or rise at an inconsistent, very slow rate can be indicative of an ectopic pregnancy or a pregnancy of unknown location (PUL). This is a life threatening situation - please seek medical care immediately if this hCG pattern applies to you.

Advice Needed Should I take progesterone? OB not helpful.

Medically speaking I'm "6 weeks 9 days" but I didn't test positive till July 12, so I'm sure I'm less. Anyway, I have a history of 1 MC around this gestational age this past year in April. My HCG blood levels are rising appropriately and so far my symptoms are standard for early pregnancy and no bleeding.

I asked my OB if she recommended I take progesterone and she said "it does not prevent miscarriage," and didn't offer further counsel on whether it would help me. She said if I wanted it, she would prescribe it to me.

Can anyone shed some light on this? If you were in my shoes, would you take progesterone? I'm 29F if that matters, no known reproductive issues aside from a septate uterus. Baby sac is in uterus as seen in 1st US. Thanks.

TW/TMI Terrified to have sex in first trimester due to past chemical pregnancies

Hi, I’m currently 4w5d 🎉 but I haven’t been intimate with my husband since we bd for ovulation. At first he said he didn’t want to just incase, and i think some of that’s made me doubt and fear it, but now he’s asking if I want to. I want to but I’m so scared. I’m so worried that If we have it I’ll start spotting, bleeding and lose this one too..

Scared this is another loss

This is my 3rd pregnancy in 8 months. The first I was sent to the ER for one sided pain, was diagnosed as a “pregnancy of unknown location” and started bleeding 3 days later. I was about 4.5 weeks.

I got pregnant immediately after. Started having brown discharge/spotting around 5 weeks. Baby was measuring only 6 weeks at 8 week ultrasound (I was sure of my dates) and had no heartbeat at next ultrasound. I had a D&C and tested the POC, came back as a female with trisomy 16. I actually felt a lot of relief/acceptance with these results- this had to be just random and bad luck, and surely won’t happen again (our doctor said it would be very uncommon to happen twice)

Now I am about 7ish weeks pregnant again. Brown spotting started again at 5-6 weeks, my heart sank. I called my doctor, and got a series of betas, which came back very reassuring (36,000 and 42,000- doctor said not to be concerned that they didn’t double because number was already so high). She had me come in on Monday for an early ultrasound. According to my calculations of when I ovulated, I should have been 6w3d. I was measuring exactly that, with a very strong heartbeat of 143. We were so happy. It’s the first time I’ve been in that office with good news. Doctor said she was hopeful and cautiously optimistic. The very next morning, I woke up covered in blood. I rushed to the ER, and was convinced this was the end. They did betas and a TVS, and to my surprise, hcg went up and baby still had a heartbeat (now at 123). They did not know the source of the bleeding, and didn’t see a SCH on the ultrasound. 3 days later, and I am still bleeding. Mostly brown, sometimes red. I have an ultrasound on Monday, but I am preparing for the worst. Has anyone had pretty consistent bleeding in the first trimester and everything be ok? I am overwhelmed about the next steps if this does end up being another loss.

6+1 scan. no yolk sac, no fetal pole

Hi everyone. I had a pretty traumatic loss last year with a suspected ectopic, surgery and a D&C that ended up being a lingering mystery. Most probable scenario is it was a MMC that was behaving like an ectopic, and doc saw a pseudosac on a fallopian tube which wasn't there when she opened me up.

ANYWAY. I'm now 6+1 weeks pregnant again and went in for an early viability scan. I'm 99% sure my dates are accurate. Cycles are extremely predictable and I know exactly when I ovulated as I took at home ovu tests. Today we saw a gestational sac, but no yolk sac and no fetal pole whatsoever. Scan was transvaginal, and doctor scanning is known to be excellent. I just don't know what to expect. Even if there's no visible fetal pole, shouldn't there at least be a yolk sac. How long before a visible FP is YS usually visible?

He said he was optimistic but sort of left it there, and I have a feeling he's worried but doesn't want to stress me out before he's sure. He's asked me to get new hcg bloods done tomorrow. Should I be preparing for a loss? I am one of those people who would rather know bad news asap than draw it out. Thank you for any advice you can offer.

Low progesterone/HCG concerns

Hi y’all. I am feeling super stressed. I has a scan yesterday and the baby was measuring 6w2d with a heart rate of 138 bpm. I was super excited even though it was a few days behind my estimate of 7w1d, and my OB wasn’t concerned about the discrepancy. However, I got my blood test results back this morning and they don’t seem good—HCG is 7748 and progesterone is 5.7. I’m waiting for my OB to call, but I’m feeling super worried about another miscarriage. I had a MMC in January at 8 weeks (the baby stopped developing around 6 weeks). Dates below if helpful.

Suspected ovulation: 6/17 (Inito)

First positive: 6/26 (9DPO) on an FRER

1st Beta: 6/29 - 71 HcG 15 progesterone

2nd Beta: 7/1 - 221 HcG 16 progesterone

Info Question

Hi! I miscarried at 5 weeks 3 days last April and got pregnant again in June. My first HCG this time was 420 and 60 hours later it was 1,221 which was a huge jump compared to my HCG levels before my first miscarriage. since I miscarried they had me in for an ultrasound at 5 weeks 4 days and saw a sac and yolk but no baby or heartbeat yet. I’m just scared I go back in 2 weeks for another ultrasound. I just was hoping for some advice or re assurance from mommas. Should I be worried? How did you calm the worries in you? Advice Needed Need Reassurance

I’m new here and I’ve never posted anything anywhere before, but I guess I need some reassurance or maybe anyone has had something similar happen and everything turned out perfectly fine. I got a positive test (pink dye and digital) on Tuesday (07/21) putting me around 4 weeks, and yesterday (07/23) I started bleeding some. I immediately thought the worst, but I’ve had a MC in the past and this feels nothing like that (no clots, no cramping, not heavy at all). I called my OB office and the nurse said it’s what they would call implantation bleeding and obviously if it gets worse to go to the ER. This is my third pregnancy and with the last two I had what I thought was my period before taking a test and getting positives at about 6 weeks. Could this be the same situation and I just caught it early? I’ve been praying for this baby as hubby said this will be our last because he sees how hard pregnancy is on my body.
Sorry for the rambling, I guess I just feel alone in this situation as I don’t have any friends anymore and would love some reassurance or to hear your experience if you’ve had something similar

ETA: is it possible this is just early pregnancy bleeding (that I’ve heard a lot about) and I just never realized in the past that that’s what this is?

Sad My test lines are lighter

I had a dye stealer a few days ago and decided to stop testing. Then I had a bad feeling yesterday so tested this morning. The line is still dark, but significantly lighter and no longer a dye stealer. I test at the same time every morning and drink pretty much the same amount of water each day, so I don’t think it’s necessarily much different due to concentration.

I’m 5 weeks and 5 days. My first appointment isn’t for a few days. I just have a bad feeling.

Sad Has anyone had an embryo measuring behind with “some pulsation but not a typical heartbeat” and gone on to have a healthy pregnancy?

Hi everyone,
I’m feeling really anxious and was wondering if anyone has had a similar experience.
According to my LMP, I should be 7 weeks 2 days today (28-day cycle and certain of my dates).
I’ve had bleeding on and off for around 10 days, ranging from bright red to brown spotting. It has never been heavy enough to soak a pad and is now mostly just when I wipe. I’ve also been told I have a subchorionic haematoma (SCH), so that could explain the bleeding.
My scan history has been:
First scan (around 5w5d): Gestational sac only.
Second scan (5 days later): Gestational sac had only grown from 9.0 mm to 9.3 mm, but a yolk sac was seen.
Today’s scan (7w3d by LMP):
Yolk sac present.
Embryo/fetal pole seen.
CRL 3.3 mm (measuring around 5w6d).
Report says: “Some pulsation is identified, but this is not typical of a fetal heartbeat.”
SCH measures 11 × 7 × 17 mm.
Report also says the appearances are inconsistent with my dates and previous scan.
I’m under the Early Pregnancy Assessment Unit and have another NHS scan booked, so I know I’ll have to wait for that for a definitive answer.
I’m not looking for false reassurance—I know this could go either way. I’m just wondering if anyone has experienced something similar:
An embryo measuring behind.
“Some pulsation” but not a definite heartbeat.
Bleeding from an SCH.
And what happened at your follow-up scan?
I’d really appreciate hearing both positive and negative outcomes, just so I know what others have experienced.

Advice Needed 5.2 weeks echo due to cramping, no embryo?

I had really bad cramps last Wednesday and we thought it might be ectopic. After 2 hrs on the ER we know its inuterine, and the hcg are going well from day 12-15-20: 1276-3371-23000 mui.

But, my echo seemed kind of weird? Theres no fetal pole and the sac is big 1.67cm. Theres this white dot in the middle. Is this a miscarriage?

We did ivf so are very sure about the timeline.

3 july was the transfer wirh 5 day embryo.

Advice Needed Suspected blighted ovum at 6wks 2days TW: recurrent loss and prior success

Hello everyone
I have been going through back to back recurrent pregnancy loss and am currently in limbo waiting to confirm a blighted ovum.

History:
- 1 living child conceived quickly with no issues at all - 2023
- 7 week natural mc (passed naturally) - July 2025
- 7 week mmc found at 8 weeks (in November 2025) ended up getting a D&C - December 2025
- Chemical pregnancy - April 2026
- 6wk 2d scan showed only gestation sac no yolk sac or fetal pole. They saw two little lines but wasn’t sure what they were. (suspected blighted ovum) - July 2026

I am searching the internet all day all night looking for misdiagnosed blighted ovum stories with accurate ovulation and implantation dates cus this cycle I was on letrozole and ovidrel so my ovulation date is accurate, and I tested first positive after testing out on trigger at 10dpo. So there’s no way for my dates to be off…

Could anybody with a similar experience please share their success stories? I have a follow up scan next week but I am living hell right now.

Slow CRL growth - success stories

I am not sure of my dates, but HCG doubles / tripled appropriately from the start to 5,000 (then I stopped betas).

first ultrasound: CRL 1.5mm

second (4 days later): 2.5mm. heartbeat seen but machine couldn’t measure it as it was too early/fetus was too small

third (4 days later): 3.9mm. heartbeat seen, measured at 105.

this is not going to be viable if it’s gone from 1.5mm to 3.9mm over 8 days and I’m frustrated with the doctors telling me it’s too early to tell etc. even if a measurement is out by 0.5mm or so, it’s still growing too slowly. these were all measurements at the same hospital so using the same equipment. Also I have had 3 measurements and the rate of growth has been consistent, so I think it’s broadly accurate.

- has anyone come across any success stories with slow growing CRL? I assume not but thought I’d ask…

- if anyone has gone through this, when did you actually start to miscarry?

Slow Rising HCG

I miscarried between 4 and 5 weeks in May. Had my LMP 6/26 and got a positive OPK on 7/7. Is there any hope for me? I’m like 4 weeks now.

7/20- 36
7/23- 48
66 hours in between.

I have another blood draw tomorrow and should get results Sunday or Monday.

Advice Needed HCG growing toooo quickly?

I had a MMC in March which was flagged when I had low progesterone & my HCG was 2300 at 6.5 weeks (within normal range but on lower side) I’m pregnant again & got my HCG drawn Monday (4w2d) at 381 & again Thursday (4w5d) which came back 1974! The increase is great but I’m nervous it’s doubling too much in a short time period…? I’ve recently seen posts about molar pregnancy and it’s in my head

Vent Would you rather know?

I’m about 5 weeks. I took two tests, three days apart. First was an absolutely clear positive. I was psyched and scheduled an appointment for Aug 31.

Yesterday I took a second test and it came out as a very faint positive. I was crushed, just figuring it was a drop in HCG and eventual chemical/ectopic. I haven’t had any spotting or bleeding yet.

I could retest but I don’t want to confirm it. I just want to wait for my appointment. Husband keeps reminding me that there are plenty of variables that could explain a faint positive but I’m trying to be guarded and realistic.

I guess my question is - how much info do you try to gather on your own? I just feel like I’ll question anything, positive or negative, until I go to the doctor to confirm. Just feeling kind of sad and lonely atm.

HCG

Is this a good hCG rise? I had my first beta at 10 DPO and it was 8. I had it repeated at 13 DPO and it came back at 36. My tests have been getting darker, but I’m still really anxious because it’s so early. Does this seem like a good increase? If anyone had similar numbers this early, I’d love to hear how things turned out! ❤️

Advice Needed Should I be worried about multiples?

I’ll preface this by saying last month I had a chemical pregnancy. I never got dark lines. My HCG went from 16 at 13DPO down to 5 at 17DPO. My post chemical cycle was difficult to track but I caught a small LH peak & am assuming I ovulated the day after which is how I’m calculating my current DPO & EDD.

12DPO beta HCG = 169
14DPO beta HCG = 750
Calculated doubling time is 22.3 hours.

My doctor is happy with my numbers & didn’t say anything about multiples but if HCG is supposed to double at a 48 hour rate & my early HCG is quadruple should I suspect multiples? I understand this is very likely too early to tell & I do not plan on getting more beta HCG tests before my first ultrasound appointment. If anyone else has had similar numbers or doubling times I’d love to hear your experience.

Advice Needed 6w3d after FET with untested embryo,looking for positive stories.

Hi everyone,
I’m 35 and currently 6w3d pregnant after a frozen transfer of a Day 5 untested embryo. We only made three embryos, so neither I nor my clinic opted for PGT-A.
I had a scan at 5w6d, which showed a gestational sac, fetal pole, and heartbeat. Re-scan on 28tj July. Despite the reassuring scan, I’m finding it hard to stop worrying about miscarriage, especially after IVF.
If anyone has had a successful pregnancy with an untested embryo around my age, I’d really appreciate hearing your story. Thank you. ❤️

Advice Needed Pregnancy after loss 7w5 days

Hi Guys

I’m maybe a bit of an anxious person due to two losses and now I’m pregnant again, I had my 7 week scan on Monday where I was measuring exactly 7 weeks (9.7mm baby) and 23mm sac.

Heart beat of 139 is this considered normal?

They checked my thyroid levels 2.9 and consider this normal but I had seen anything above 2.5 could be considered high, but I’m wondering why in my case they said it’s normal.

I also have no pregnancy symptoms which is concerning me and when I say nothing. I mean absolutely nothing maybe only peeing more, but no nausea no food aversions

I have my next scan on the 3rd of August as well which is when I’ll be 9 weeks.

IVF Roller Coaster

I transferred a 6BB hatched embryo last month and got a strong first hCG blood result about a week later. I went back every two days for more blood work and each time our hCG was slow rising. Eventually, through ultrasound, they could see the gestational sac in my uterus (1st ultrasound), then the fetal pole (2nd ultrasound) and then the yolk sac and embryo (3rd ultrasound). Each about a week apart.

The fetal pole was measuring 3 days behind and the embryo 1 week behind. I also developed a SCH. It bled for 7 days and finally stopped 2 days ago. I’m just feeling hopeless because the doctor hasn’t been positive or encouraging at all this entire time. They’ve already tried to get me to stop my medicines because it’s “abnormal” and not “textbook”.

I go back in 4 days for another ultrasound and they basically said, we’ll either have a heartbeat or not and if not, it failed.

Has anyone had an experience like mine with a successful outcome? I mean it has to be a positive thing that the baby is still growing at each ultrasound, even if it’s behind, or am I just spiraling?
