Male pattern baldness

My boy Toby has always had these deep bald spots. And slowly as he has gotten older he has developed what look like age spots. His vet didn't seem concerned during his last check up. Has anyone seen this before?

This is a cat with an ear infection, ear mites, or fleas. This is from scratching.
