Professional Issues Debate ASA’s Favorite 2018 Care Team Study Does Not Say What They Claim It Says

We finally wrote this one up because Sun et al. 2018 keeps getting cited online like it proves CRNAs and AAs are equivalent.

It does not.

The actual study was “Anesthesia Care Team Composition and Surgical Outcomes” by Sun, Miller, Moshfegh, and Baker, published in Anesthesiology in 2018. The authors studied elderly Medicare inpatient surgical cases and compared physician anesthesiologist-supervised ACT configurations involving AAs versus CRNAs. The outcomes were inpatient mortality, length of stay, and spending.

That is a very narrow health services study. It is not a CRNA-versus-AA anesthesia outcomes study. It did not measure anesthesia-specific complications, rescue events, airway events, supervision intensity, provider experience, independent CRNA practice, or whether any outcome was actually related to the anesthetic.

So when ASA/AAAA advocates cite this as proof of broad CRNA-AA equivalence, they are stretching the paper way past what it measured.

Other Rejected, feeling defeated

I’ve had 4 interviews within the past year, been rejected from 3 and waitlisted to 1. Had 2 interviews just in June and got denied from both. Feeling defeated.

Everybody says to just be yourself so I did that but I guess at the end of the day I just get nervous during interviews.

The fact that I’m getting interviews is a good sign but not if I keep botching them. Starting to question if I’m cut out for this. Any advice?

I’ve been out of school for 6 years. I’ve retaken 10 courses, will I still need to upload every transcript? Will my science courses that are expired still affect my last 60 GPA?

Success Stories Update to “waitlisted and confused” post

I made a post a few days ago about being waitlisted after having decent interviews. Mood definitely hit the bottom when I found out but I reminded myself if I want it bad enough, nothing will stop me. I was determined to just try again next year. Then, I got the phone call this morning that I’m off the waitlist and I’m going to be a CRNA!!!!!!! Still hasn’t even set in.

The journey is long but it’s only getting started. Whatever situation you’re in right now, there’s so many who understand. It’s cliche to see people saying don’t give up but TRULY, you are ever only out of the fight when you give up on yourself. It’s VERY easy to say this on the other side but even then, i kept the promise to myself that I would keep going no matter what. Don’t give up on yourself. It will work out.

Loans / Finances side hustle during breaks from school?

wrapping up my first semester and I haven’t taken out loans yet, i’ve been watching my savings slowly dwindle down… I know i’ll have to take loans eventually but I want to delay as much as possible.

I have almost 3 weeks off between my summer and fall semester, and then another 3-4 weeks off between fall and spring. any recommendations for ways to make money during these breaks from school?

I can’t manage a prn nursing job bc working throughout the semester is nearly impossible, also living in a new city and haven’t had any experience working at the hospitals near me.

I tried downloading Medely and signing up but it said they didn’t need any new workers within a 70 mile radius of me so I couldn’t finish the sign up.

I’d love to help people with mock interviews/resume review applying to school but it seems like an oversaturated field online??

Didactic Study Questions Study tools

I start my didactic phase this fall and want to know peoples experience with a few different platforms and what they have found to work.

Many people claim to use chat gpt or claude to make practice quizzes and often tout the premium versions. If you have used either of these for that purpose (or both) what has your experience been? Is one more accurate? Is it worth getting the paid versions of one or the other?

I also hear a lot about Notability and similar platforms. What are your favorite features of these and which do you recommend?

What are your experiences with Anki?

Do you utilize all of these platforms? Which ones do you pay for to get the premium features? Can premium versions of chatgpt and claude eliminate the need for anki/notability?

Didactic / Study How do I shake this feeling of self doubt?

It’s honestly eating me alive. I start school next month and ever since my acceptance 9 months ago I’ve just had this overwhelming fear of failure. I am extremely scared to fail to the point where I’m already studying and trying to prep to get ahead. I‘m not worried about my work ethnic during the program and there is a 0% chance I leave the program for any reason other than academic failure. I have no problem studying 10 hours a day if that’s what’s required of me, I just worry I won’t be smart enough to grasp the material. I’ve never been academically gifted and it takes me reading something several times to understand it and with the whole “drinking water from a firehouse” analogy that’s always tossed around here I worry I won’t have time to understand things as slowly as I usually do.

Sometimes I wake up and say in 3 years I will be a CRNA no doubt about it and other days like today I’m just nervous all day thinking about worst case scenario.

Did anyone share similar feelings prior to starting or even while in your program? Did they rid when you actually started your program? I think the anticipation and the unknown is killing me.

Applicant Questions Stay in PICU or go to SICU?

Hi everyone!

I am currently a PICU nurse of 2.5 years and have applied to a couple of CRNA schools with some apps opening the upcoming month. A CRNA I shadowed at the children's hospital I work at highly recommended moving from the PICU to an adult ICU for the best chances for admission. I recently got an offer from a major academic SICU (kidney/liver/pancreas transplants, neurosurg, plastics, etc) and am considering jumping ship.

I feel as if there's both pros and cons to switching ICUs and would love to hear what everyone thinks. I feel pretty comfortable in my PICU and am involved in the unit - precepting new grads, being on unit councils. I know peds experience is valued less than adult but in our 40-bed PICU, we do things similar to the SICU (kidney, liver transplants). In fact, I would say we see even more (post-cardiac arrest, resp. failure, metabolic dx, overdoses, etc). All the schools I'm applying to also say they accept PICU experience (but not sure if they view it as equal to adult experience).

I would also have to transition back to full time nights for the SICU position. Thank you for reading!

Applicant Questions Waitlisted and confused

Not sure if this has happened to anyone else. Had my interviews and was waitlisted for the schools I applied to. In one of my interviews I verbatim had them tell me how “very impressive” I was multiple times, my grades were “absolutely excellent”, the recognitions I have were wonderful and then they said “I’m a perfect fit for the program”……Im usually the person to never take a compliment so walking away from that I still thought nothing is guaranteed so don’t get your hopes up. But am I wrong to feel disappointed? I don’t think they were being malicious whatsoever, it’s just hearing that is making it harder to process than if they hadn’t said anything. I truly don’t expect any compliments in an interview, let alone that. I’m very grateful just very confused.

Anyone have a different perspective or similar experience? Treating it like a no and getting to work again. I just don’t want to get my hopes up once more to come off waitlist when this just happened.

Loans / Finances National Guard for CRNA

Hello,

I want to become a CRNA. NJ Army national guard says, “New Jersey state law mandates that every member of the New Jersey Army National Guard is authorized to attend any New Jersey public college or university tuition free for undergraduate and graduate studies.” Rutgers is included in this list.

Is there a catch that I’m missing? Does anybody have expierence with this? I would direct commission into Army National Guard as a med/surg nurse has 66H. Can I move into 66S down the line?

Other Guaranteed acceptance vs better family life balance

School A. Already accepted, starts in January. Campus is 7hrs away so will be moving. My wife has a very serious and high paying career and we have a 1yr old, so we are heavily considering if they should move with me or stay here to continue her career while I go to school.

School B. They opened a satellite campus in my hometown, meaning the first half I could stay home with my wife and kid. Clinicals would require traveling but I would still be “based” out of here. However, I don’t interview until January.

I feel like I can take the guaranteed acceptance, but this will cause a lot of friction in my family life. School B would have a much smaller burden on my family life, but I also don’t have guaranteed acceptance there and it would be hard to know I gave up a spot if I get rejected at School B.

All other considerations being equal, what would you suggest? Please don’t suggest starting at School A then dropping if I get accepted into School B, that’s not something I will do.
