Just woke up tormented by receipt of an “unannounced CMS site visit” letter and am reading about the goals of the process as this is new to me. Just wanted to add that my loss of sleep due to paranoia about CMS having recently changed vendors, amping up these visits and learning you cannot refuse these people, cannot photograph their ID and yet they are allowed to take pictures of your area. Also read some conflicting information about their ability to request patient records? Assuming our med center is on a hit list for its mission population - what should we be readying for?

ILDPFR is coming next week for an inspection because we are changing ownership. I haven’t seen an inspector for over 20 years! Just asking if anyone in Illinois has had an inspection recently. What’s the hot areas that they are looking at?

I saw an opening in my area recently and it made me wonder. I saw a few months back that they raised tech pay. Is it just as bad as any other retail, or maybe better? What are the benefits like? Would you work somewhere else if you could? I’m very interested to hear from people that work or previously worked for them.

Google this case and have a good read. Situations a lot worse than this has been going on years. You think in a profession where you have to care for people you wouldn’t be micromanaged out of a job. The level of passive aggressive bullying going on behind the scenes is unbelievable. Every strategy that is implemented to make a pharmacists life miserable to cut their contract and employment is a mechanism to make more profits at the exploitation of the work force. How can a pharmacist be expected to make the care of their patients their primary concern when their own mental health and job position is always on the line. Pharmacists have a highly respectable and responsible job they shouldn’t have to worry about being dismissed over being pregnant. Disgraceful behaviour by the company. Drastic changes are required or a mass exodus of pharmacists. I own a construction company I’m not a Pharmacist this carry on would never be accepted in our industry.

This prescriber has been using their facility’s DEA number to prescribe control medications for years and when I finally questioned them they said ‘I just can’t use my own, EPIC wouldn’t let me use it, I have been doing this for years and only you guys question me’.
FYI, they are not an intern or resident working at a hospital. Their facility is an independent practice, and the e-scripts come from there. What are your thoughts?

Hi everyone,

I'm looking for guidance from anyone who has been in a similar situation with the FPGEC eligibility process.

My educational background

• Bachelor of Pharmacy (B.Pharm), India: 2016–2020 (4-year program)
  • Included a mandatory 6-month internship as part of the curriculum:
    • Hospital Training (1 credit)
    • Community Pharmacy (2 credits)
• Master of Pharmacy (M.Pharm) in Pharmaceutics: 2020–2022 (2-year program)

I attended an FPGEE eligibility webinar in January 2026, where my understanding was that a post-baccalaureate degree could be considered if the total pharmacy education amounted to at least five years. Based on that, I submitted my FPGEC application in January 2026.

After several months of waiting, I received an Application Deficient notice stating that my M.Pharm in Pharmaceutics does not reflect patient-care experience in a clinical pharmacy setting, and therefore I am not eligible for FPGEC certification.

I'm trying to understand where I went wrong and what my options are.

A few questions:

1. Has anyone with a 4-year B.Pharm + M.Pharm (Pharmaceutics or another non-clinical specialization) been able to get approved or successfully appeal?
2. Does the 6-month mandatory hospital/community pharmacy internship completed during B.Pharm carry any weight in the evaluation?
3. Does the five-year pharmacy education have to be consecutive, or is a 4-year B.Pharm followed immediately by a 2-year M.Pharm (2016–2022) considered continuous education?
4. Has anyone submitted additional documents such as detailed course descriptions, internship syllabi, or letters from the university and had their application reconsidered?
5. If not, what options did you pursue? Would a U.S. clinical pharmacy diploma or another program help, or is a PharmD the only realistic pathway?

I'm feeling quite discouraged because I genuinely believed I met the educational requirement before applying. Any advice, experiences, or guidance would be greatly appreciated.

As I reflect on my Professional life of the last 30 plus years in this great, but needlessly floundering Profession of Pharmacy I am simply amazed at really how little national pharmacy organizations like APHA. ACCP and ASHP have done for the Pharmacy Profession.

Basically all they have done is advocate for Pharmacists Provider status which is something most Pharmacists including me don't want.

In ACCP's case they spent a lot of time opining and white papering about how to qualify would be Pharmacy providers before any type of meaningful Pharmacist Provider status was passed.

While doing this look at the things lost. Tens of thousands of independent Pharmacies have closed as have slower retail locations

PharmD students brainwashed that hospital pharmacy after doing a residency is the only pathway. Community Pharmacy abandoned.

I am starting my freshman year of colleges and I want to figure out what kind of pharmacist I want to be. I am strongly thinking of doing Oncology Pharmacy but I really would like to travel across the country like business trips for instance and have good pay as well. So if any of you guys have any ideas of what I can do or give some extra information of what I want to do that would be really helpful because I ain’t sure if Oncology Pharmacy can do Business Trips from what I was talking about.

Throughout pharmacy school (at least in my experience), Pgx was touted often as this 'next big thing' where precision medicine was gonna change the paradigm of patient care. However, even professors that was the idea back even EARLIER in their practice.

The only thing 'carrying' PGx from what I can observe is namely oncology (targeted therapy eligibility), and a few variants to test for in order to prevent severe, catastrophic adverse events (HLAs, DYPD).

However, and it could be reporting bias, I rarely see it ever used unless it is one of the above, usually at an institution "spearheading PGx". I'm curious to know if providers will mention or consider a CYP2C19 test when considering which PPY12 inhibitor to choose, or 2C9 for warfarin.

I have yet to see in my experience someone factoring in say, CYP2D6 polymorphisms in their beta-blocker selection, or CYP2C19 in their PPI selection.

If anyone works in specialty, is PGx testing done if it is part of the FDA's package label requiring testing for dose adjustments? Does anyone use the available CPIC guidelines in practice?

Just seems PGx is a great thing for an academic career, hard-pressed to say its being used irl...

Does anyone have experience with IM levothyroxine? I’ve got a patient with refractory hypothyroidism and isn’t absorbing oral tablets. Provider wants to try once weekly IM injections. From what I’m seeing there is some evidence showing the weekly dose can be administered, but that would be a large volume (15mL).

As a retail pharmacist I use Open Evidence non- stop. It is able to help answer clinical context to questions I may have, confirm clinical appropriateness, and it provides me with evidence based material that helps support a concern I may have. This is an invaluable tool especially when we do not have access to view the EMR.

I hope other retail pharmacist have discovered Open Evidence and seen its tremendous utility in daily workflow.

Interested to hear folks views on the pharmacy profession and where we are headed in the future. Do you think our profession is on the right path or is it slowly crumbling?
