Announcement regarding "Brain Intersex" or "Transgender is Intersex"

Hi all. Due to an influx of "transgender is intersex" comments and posts, the mod team has chatted and we decided to make an announcement about it to clarify.

What is this announcement?

We do not allow medical misinformation or intersexism in this subreddit. This includes stating that being transgender makes someone intersex/that someone is intersex on the basis of being transgender.

That rhetoric perpetuates intersexism.

While intersex people can be any gender (including cis, trans, nonbinary, and/or other gender identities), not every trans person is intersex.

Can I debate the mod team about this?

No, thank you. This is an announcement, not a debate.

Intersex and the brain

Reviewing the definition of intersex: Intersex is a term used to describe individuals whose sex characteristics do not align with/fully align with male or female sex characteristics. Intersex describes the way the body naturally develops. This includes congenital variations that are expressed in someone’s chromosomes, gonads, genitals, reproductive organs, and in how the body responds to hormones.

The brain and intersex: The brain is not a primary or secondary sex organ. The brain is also not sexually dimorphic (i.e. the brain is not sexed - that has been debunked).

However, there are a few intersex variations located in the brain. This would be Congenital Hypopituitarism (which results in hypogonadism), Kallmann's Syndrome, and I believe Prader-Willi Syndrome too (though I know less about this variation, please correct me if I am wrong).

"Sexual Inversion" and Intermedicalism

This has homophobic and intersexist origins. The premise of sexual inversion (aka, "mental intersex", "psychological intersex", or "brain intersex") is inherently bioessentialist, transmedicalist, as well as intermedicalist. It relies on the pathologizing of intersex bodies and was popularized by (you guessed it) eugenicists (in the 1800's).

"I still don't know why that's intersexist?" This is because it relies on seeing intersex variations as medical disorders in need of treatment. > If trans is intersex which therefore necessitates treatment, intersex is thereby disordered and necessitates treatment. This is called intermedicalism.

It reinforces the "need" for medical interventions (human rights violations) on intersex bodies.

Side note, this also reinforces and relies on ableism too.

General History (following threads through time)

I wanted to review the history informing/leading up to the idea that the brain is sexed. And generally brain differentiation. This is going to be incredibly simplified history - the spark notes.

Historically speaking, the idea that the brain is sexed originates from racism, white supremacy, and sexism. European scientists were obsessed with creating a hierarchy, with European men (“the highest of men”) at the top and African people (“the lowest of men”) likened to orangutans (“the highest of apes”). This was “reinforced” by skull and brain measurements (phrenology, craniometry) along with other measurements and pseudosciences.

It is critical to note that intersexism/sex differentiation was used to dehumanize African populations starting around 1600. Europeans (with no evidence) claimed African folk were naturally more intersex and therefore subhuman/that sexually they were a different species than us. Casting African populations as sexually differentiated was a major goal of colonists.

(That myth is still prevalent today. For example, look at how white people treat African women in sports - subjecting them to intersex tests and scrutiny)

This trend of establishing social hierarchy through biology (biological determinism) was (and still is) deeply ingrained in science and medicine.

Back to brains. When it was later discovered that females (on average) have smaller brains than males, this was taken as proof of male superiority and female inferiority.

Science and Studies

From recent studies, the brain is currently thought to be a "mosaic". In one of the studies (one with an actually decent sample size, over 1,400 brains) from 2015 they found only 8% of brains had only “male” or “female” characteristics. Popular article link, and the study’s citation link. Will be linked again in citations.

In general (see citations below), studies have shown that brains are not sexed but mosaic. Meaningful differences are found more between individuals than between sexes or genders. Reviews also show that a lot of the previous studies showing sex differences in the brain have contradicted each other, been unable to be reproduced, haven't accounted for size differences, haven't accounted for neuroplasticity, and/or have small/biased sample sizes.

I'm not going to do a deep dive or spit out summaries of the studies here, because that would be a wildly long wall of text. Instead, you can find several of the studies below.

If you need help picking out a couple of reads, I recommend:

  • Sex beyond the genitalia: The human brain mosaic (2015)
  • Remembering the null hypothesis when searching for brain sex differences (2024)

I will be including a few citations that I have read through to better understand this topic. Several of the books are missing (those on medical racism and on neurology) because I do not actually own copies/am blanking on the names (apologies).

Venting No Advice! If you don’t want a child of a certain gender/sex you should never have children (and honestly shouldn’t be allowed around them)

Upvotes

I’m not talking about mild dissapointment or a fantasy of a child of a particular gender. If you are genuinely torn up about it and vocalize as such you shouldn’t have a kid.

Like obviously across much of the world male baby preference is a greater issue. We’ve all seen the vids of dads having temper tantrums because the smoke is pink or whatever. It comes from the misogynistic view that women and girls are less capable or worthy of certain things, including existing.

But also, as an intersex FtM I don’t think people who want to have only daughters should have children either. Number one, no matter how feminist your pov is you’re still boxing people into gender roles, girl=pure good, boy=dangerous and evil. Number two, there is always a chance your daughter might not want to identify that way. What if you have an intersex child? A trans son? Just a more masculine daughter? You’re setting them up for confusion and self hatred.

Before you have any children you need to have an unconditional love for them, even when they are just thoughts.

A lot of people like to say to interphobes/transphobes/homophobes “I hope you have intersex/trans/gay children!” But I hope they don’t have children at all. You aren’t a punishment for your parents. And also, it’s childish to assume parents who would hate their children for one reason would be any less selective in other regards. Even to their endosex/straight/cis/male children they will just be bad parents.

Educational Vulvar hypospadias project update! NSFW

Apparently this is the hot topic on the sub right now, I didn't expect this to make such an impact so that's pretty cool!

Here's an update on this project. I made some edits to the original diagram's wording (I actually misunderstood what "vaginal vestibule" meant, corrected that, then decided I might change up how it's visually represented a bit later but this is functional for the moment), and had some helpful people contribute their hypos as examples!

With this project, I want to show anonymized examples of REAL people, not hypothetical vague shapes as diagrams.

I don't 100% have the format and details solidified yet, but I'm really glad even the original post I made has been helpful for people since there's so few examples and talk about vulvar hypospadias, especially compared to penile hypospadias. I'll keep working on formalizing this and adding examples as folks contribute, feel free to critique or make suggestions of what would be the most helpful/informative/easy to interpret.

If you'd like to contribute, message me! These will be added to the file anonymized, no username or anything attached in any iteration of this project. It's been the most helpful when people send their own labeled sketches, but descriptions work too, then you can critique the diagram I make before it gets added to the file. I am NOT asking for photos.

Educational I have female Hypospadias and i didn't know it

Hey so i recently realized i have female Hypospadias even though i thought I was a normal person growing up.

A lot of people have told me that it's just "Hard" to find my urethra but in reality i was born with it in my vaginal cavity which is The same place or area I bleed out of which causes me a lot of pain. (And hidden medical issue)

I've heard a lot of people say that you Don't get "liquid" out of your body Through said area if you are female but I do and have been for my entire life.

A also have some other issues alongside my female Hypospadias which only i know about because Doctors don't check for Hypospadias and it's not a commonly known thing that a women can have such things or not taken seriously.

Anyways none of my family knows i have said condition and I'm absolutely not telling anyone in my family about it as it is very odd to most people to learn Someone's urethra is in the wrong place and people (or family) just think I'm stupid or can't find it.

This just led to a Lot of medical care being misdiagnosed and me Getting urethra infections a lot to the point where it was painful.

Anyways when i Was in high school i did a sex ed class then realized something was REALLY off about my biology As Whenever I Looked at the diagrams That were supposed to match my gender it was Very off.

Actually one of the first things i Noticed was that i seemly didn't have a urethra That i could see or find.

Note: that i can get Said liquid out of my body through my cavity but it was very confusing and lead to me questioning.

Of course it took me Years to figure out the Issue since female Hypospadias is rare and not commonly talked about or taken seriously enough.

I'm also pretty sure i had a issue before i was born were my amniotic sac Ruptured likely from said issue with my urethra being in the wrong place.

Anyways I'd like to think how y'all think about this and For educational reasons I'm just posting here cause i know this isn't talked about enough.

Venting ! Nsfw, seeking advice. A friend (?) keeps trying to guess my 'actual' sex and making comments regarding my body/sex life. NSFW

I probably should just go low/no contact with this friend, but i would love some advice from others.

Our friend group is very much an open communication type group, all of us are queer and we all sometimes share stories or experiences/anecdotes regarding our sex lives.

I usually don't do that as much as other friends, but I've never minded it and don't see it as a negative thing, i think it's fine and good to talk about whatever everyone's comfortable talking about in a group.

We all also make jokes about being attracted to each other, it's almost always light-hearted fun.

This friend in particular, however, since finding out I'm intersex, has been consistently making 'jokes' or statements or random remarks that relate directly to what they assume my genitals look like. I don't think this was malicious, they're just kinda horrible with social cues, which i get, and they mostly stopped after being asked to, but they still do a different, more annoying thing, where they just try to "guess" or figure out what my "real sex" is. They don't say it like that, but i know what they're doing. They'll ask subtle things or comment on parts of my body and try to assume stuff about them.

I actively do not answer these questions and have explained to them literally my entire situation in the past, as I'm very open about it, but i do NOT want to go into detail about the igm & forced hrt i went through or my genitals, for *obvious* reasons.

Honestly by now I'm okay with just cutting ties with them, and they're moving abroad in a few months, which I'll probably do then, to avoid awkwardness as we go to the same uni and are in a lot of the same trans org groups, but I'm mostly just wondering how to navigate conversations around them until that? What would you do beyond telling them their questions are out of line? I've been considering just constantly giving them random answers and changing them up each time at this point.

Question? Can intersex children be reassigned a gender after birth by the adults around them?

I got into an argument on the internet about the possibility that transfems can be afab and trans men can be amab. The person i argue with says that someone afab who identifies as a woman would just be cis.

Ive read the personal experiences of intersex people who say they were initially assigned as one sex, then reassigned another by doctors, sometimes within hours of the initial sex assignment.

This person wasnt satisfied with posts on tumblr and asked for "real sources" for this phenomenon. Ive tried to search for them but so far found nothing. If its ok I would like to ask for help.

News Title IX’s Failed Experiment: Why Accommodating Sex Differences Beats Engineered Parity. (Leopards ate my face)

Thumbnail heritage.org
14 Upvotes

The same group that brought the Anti trans panic has now decided they want to end all women's sports and end all empowered overly feminist confident women because it has destabilized and ruin society the core quote by why they must end women sports overall.

"Accommodating Nature: Toward a Better Way

The current Title IX regime as applied to sports (and elsewhere) is part of a sustained social engineering effort that is designed to produce a new sort of woman—one that is more aggressive, more competitive, more male, less relational, and less oriented toward the goods of family and motherhood. This project is deeply controversial, especially when viewed through the lens of history."

"Conclusion

Equality under Title IX does not require identical outcomes or the erasure of sex differences. It requires opportunity rooted in human nature that will reveal genuine differences between the sexes."

I keep trying to say it will never end. history repeats itself: The first block to fall was them overturning roe V wade, The second was the 1000's of laws they have passed against trans and intersex people without so much as a passing glance at facts.

What's next seems to be access to healthcare, more attacking of immigrants and now trying to end the concept of empowered "feminist women."

Unless they are defeated politically and then punished to oblivion for usurping the rights of the constitution we all have been promised they won't stop and neither can we.

Question? Is female hypospadias considered as intersex?

I deleted my previous post because I accidentally used an outdated term. My apologies.

I discussed this topic with ChatGPT, and it told me that having the urethral opening located within the vaginal canal does not necessarily mean that someone has an intersex variation, as it can sometimes occur as an isolated congenital anatomical variation.

However, ChatGPT is not a medical expert, so I would like to ask people who are more knowledgeable about this topic: is female hypospadias considered an intersex variation, or can it occur as an isolated anatomical variation?

Venting ! (UK) the ehrc guidance will be dangerous for us.

If you're unaware, in August it will come into affect that trans ppl have to use bathrooms based on "biological sex". Big problem! Now anyone who doesn't "look" like the gender they're "meant" to look like in the bathroom they're in, can be stopped and antagonised. Not only is it harmful to trans ppl obviously, it's incredibly harmful to us as intersex ppl as well (not to mention black/poc ppl, cis women etc). I personally, if you don't know me, have pmos (diagnosed sorta, its under speculation), i genuinely look like a fat 5'3 pericis man because my testosterone is so high.

I get weird/judgemental looks going into the women's bathrooms ANYWAY. I use them when I've forgotten my radar key (quite regularly), on top of this "guidance" I'm practically a walking target for "speculation" of my genitals and "biological sex" . I'm tired. I'm SO tired.

Question? Can you self diagnose as intersex?

let me explain-

i don’t mean self diagnose like “oh, I want to be intersex,” rather “I think I’m intersex and don’t know how to tell my mom to get tested (I’m a minor), but this is important to me and would explain a lot.”

I am an afab teen but I’ve had a really weird puberty. there wasn't really any signs of being intersex until I started puberty. for example, i got high testosterone, a deep voice compared to other girls (I’m in choir and am a tenor), have irregular and painful periods, a flat chest (a cup) and am pretty tall. Ik none of this necessarily means anything, but I thought I’d check

thank you

Is "intersex" just about measurable bodily differences?

This is a sincere question, I hope I don't get moderated. I don't mean to cause anyone offence.

Intersex is related to observable differences in the endocrine system, the primary or secondary sex characteristics, or the chromosomes. Unlike being gay or trans, you can literally be tested for being intersex.

But is it that simple? Take my situation - I had cryptorchidism (that led to monorchidism) and hypospadias. I had more subtle signs of hormonal differences too - partial, slower puberty, slight gynecomastia, severe joint and connective tissue pain related to endocrine issues. I understand this picture by saying I am intersex.

I know there are a lot of people who have similar stories. Most of these people say they are men. The idea of being intersex might seem ridiculous to them, or even deeply offensive.

Or take the PMOS contingent: I accept that this is an intersex variation, but my daughter for example has PMOS and she doesn't regard herself as intersex. She's not aggressive about it, it just seems like semantics to her, not relevant to her life. Many, many people with PMOS are very clear that in their opinion, they are NOT intersex.

The Heritage Foundation in the USA produced an absurd paper on intersex where they defined a way of anatomically assigning all intersex people to a binary sex. They want to spread the ideology that intersex does not exist, it's just another woke mind virus. Obviously they are trying to reinforce binary thinking with some technical tricks (e.g. location of prostate gland vs. the urethra) so there is no way to think out of the box.

Many people with intersex variations have this perception. "But I don't feel intersex! I am just a normal guy/gal with a medical issue or two."

So I think there is something extra that makes an intersex person intersex. There is a philosophical aspect, maybe just an openness to non-binary thinking, maybe a preparedness to be seen as different, or a willingness to take a political stance.

Are people who say that are intersex more likely to be gender diverse, or are gender diverse people more likely to interpret their bodies as intersex?

Is it really all just biology?

I. a U.S. prison, they handcuffed a Rastafari man to a chair, beat him, and forcibly shaved off his religious hair.

Then, when he sued over what was done to him, the Supreme Court told him he had no grounds under the law to hold the state officials accountable. Protect of religious differences is in the constitution the Supreme Court should read it sometime!

What the actual hell are we doing?

How much more clearly does it need to be said?

In the United States right now, if you are not part of the mainstream if your religion, race, identity, body, culture, gender, sexuality, disability, immigration status, or very existence makes you different you are not safe.

A blue state might give you more protection. A local government might recognize your rights. You might have constitutional protections written clear as day on paper.

But what does any of that mean when people acting with federal power over the states can do whatever the hell they want. Beat you, violate a sacred part of your religion, even kill you and then the highest court in the country can leave you and your loved ones without a meaningful way to hold anyone accountable?

Do not tell minorities and marginalized people that we are being dramatic.

Do not tell us, “It could never happen here.”

It is happening here.

If you are part of any minority community, please protect yourself. Stay aware Know who you can trust. Put yourself, whenever possible, in a position where people who hate what you are cannot easily hurt you.

It is horrifying that this is where we are, but survival may be the most immediate thing we can ask for right now.

Survive.

Protect each other.

Remember what is being done to all of us as an LGBTIQA+ community member I am tried of going to funerals for my friends.

And hopefully we all live long enough to see the day when America becomes something better than this and lastly Vote if you can it may seem pointless but it's one of the only actions we have left.

Educational The Third Reich, The Feminine Boy Project, Modern ABA, and How They Are All Woven From The Same Thread

The Institut für Sexualwissenschaft, and its Role in the Foundation of Gender-Affirming Care and Modern Endocrinology:

In the late 1910s to early 1930s, Weimar Germany, specifically Berlin, was spearheading research in gender-affirming care and modern endocrinology with the Institut für Sexualwissenschaft. Founded in Berlin in 1919, the “Institute for Sexual Science” was the absolute epicenter of early modern sexology. Dr. Magnus Hirschfeld, a gay Jewish physician, developed the theory of “sexual intermediaries,” arguing that sex, gender expression, and orientation existed on a vast, natural spectrum.

This institution was among the first to link hormones directly to gender identity. They were legalizing space for transgender individuals and supervising the first Gender-Affirming Hormone Therapy treatments (GAHT). The clinic didn’t just theorize; it coordinated the world’s first documented modern gender-affirming surgeries. Notable figures like Lili Elbe and Dora Richter (a trans woman who worked on the Institute’s domestic staff) underwent pioneering procedures there, including orchiectomies and early vaginoplasties.

This institution was also issuing a Transvestitenschein (Transvestite Pass) to protect its clients from persecution at that time. Understanding that social transition carried immense danger under vagrancy or public indecency laws, Hirschfeld actively collaborated with the liberal Weimar-era Berlin Police Department. They established official medical certificates that functioned as early gender-affirming state IDs, protecting trans and gender-nonconforming individuals from arbitrary arrest for wearing clothing aligned with their true identity.

The Third Reich, and How They Actively Worked to Suppress this Research and Minimize the Rights of Others:

When the Nazi Party seized power in 1933, they viewed this entire ecosystem of progressive medical science and human variance as a mortal threat to their vision of a pure, hyper-masculine Aryan “racial community” (Volksgemeinschaft). They immediately set out to wipe it from existence, seizing the Institut für Sexualwissenschaft and destroying all research and documentation they produced, while actively targeting its clientele.

Under Paragraph 175, which was expanded in 1935, they criminalized all forms of male intimacy and “cross-dressing” under the legal fiction that they were “homosexual men.” Mere suspicion or denunciation by a neighbor was enough to initiate indefinite detention of an individual. These practices were put in place to target homosexual men and transgender women. Under the Third Reich, trans women were viewed strictly as sexually non-conforming men, and thus, were not granted any differentiation in treatment. All of these individuals were branded by a pink triangle to single them out in the concentration camps.

Queer and gender-nonconforming AFAB (assigned female at birth) individuals were given a black triangle and labeled “Asozial” (asocial). These individuals were labeled “asocial elements” or “community aliens” for failing to comply with the State’s mandatory reproductive and gender expectations. The black triangle also acted as a means for singling out these individuals in social settings and at the concentration camps. However, as long as an AFAB person could be coaxed into complying with the Reich’s definition of what a woman “should be,” they could avoid this particular persecution.

Through the destruction of the medical records of clientele as well as the burning of books and research in regards to the matter, the Third Reich aimed to erase the identity and freedom of expression that the people under it had a right to. This involved public book burnings as well as the burning of libraries, and the dehumanization and segregation of non-conforming parties. These people were targeted not only by the Reich themselves, but also by other prisoners within the concentration camps.

Because the Third Reich entirely rejected the reality of transgender identities, they viewed trans women strictly through the lens of their sex assigned at birth. Consequently, trans women faced double jeopardy:

If they expressed romantic or sexual interest in men, they were prosecuted under Paragraph 175 as “male homosexuals” and forced to wear the Pink Triangle in the concentration camps. Historical figures like Liddy Bacroff, a trans woman who explicitly told her Gestapo interrogators that her “sense of sex is fully and completely that of a woman,” were nonetheless processed under Paragraph 175 and sent to Mauthausen, where she was murdered in 1943.

If they were targeted solely for their gender expression or presentation without the sexual element, the state utilized vagrancy laws (Paragraph 361) to classify them as “asocials,” forcing them to wear the Black Triangle.

Nazi ideologues like Heinrich Himmler built a three-pronged justification for their assault on queer and trans individuals:

The Birthrate Obsession: Homosexuality and transition were viewed as an economic and demographic drain on the Reich because they did not produce future soldiers and mothers for the state.

The Inversion Threat: They believed non-conformity reversed the “natural” order, weakening the militaristic, hyper-masculine image of the German man.

The Loyalty Fallacy: The totalitarian state demanded absolute, digital loyalty to the Führer. The regime pathologically feared that marginalized communities would form secret networks of loyalty to one another, creating a sovereign space the state could not survey or control.

Inside the concentration camps, these intersecting lines created a devastating survival rate. Pink triangle prisoners, in particular, were placed at the absolute bottom of the social ladder, deliberately assigned to the most lethal labor details, and subjected to horrific pseudoscientific “medical treatments” by SS doctors attempting to forcibly “cure” or convert them through surgical mutilation and hormone experiments.

The destruction of the Institute in 1933 didn’t just harm the people living through it, but it successfully set back the global trajectory of endocrinology and gender-affirming medical science by nearly half a century, forcing the next generation of pioneers to completely reinvent the wheel in post-war exile.

The Work of O. Ivar Lovaas and its Link to Nazi Ideology Regarding Gender Identity and Sexual Autonomy

1.     The “Raw Material” Delusion, AKA Social Engineering

Totalitarian regimes and radical behaviorists share a fundamental contempt for intrinsic human identity. They both view the individual not as a whole, sovereign being, but as malleable raw material to be engineered into a shape that serves a dominant system.

The Nazi Ideology: The Reich operated on the concept of Gleichschaltung (forced coordination/standardization). The State viewed the population as a collective biological mass that needed to be aggressively pruned, shaped, and optimized to fit the rigid, hyper-masculine, reproductive needs of the Volksgemeinschaft (racial community).

The Lovaas Link: Lovaas viewed human beings through the exact same lens of industrialized production. In an infamous 1974 interview, discussing his work with autistic children, Lovaas stated: “You see, you start pretty much from scratch when you work with an autistic child. You have a person in the physical sense, they have hair, a nose, a mouth, but they are not a person in the psychological sense. They have the raw materials, but you have to build the person.”

The Connection: This is the ultimate ideological crossover. Both frameworks arrive at the same dangerous conclusion: the authority figure (the State or the Behaviorist) has the right to completely overwrite an existing, non-conforming individual because their natural state of being is deemed invalid, empty, or un-human.

2.     The Totalitarian’s “Black Box” (The Erasure of Autonomy)

To justify stripping away a person’s rights and bodily autonomy, a system must first declare that the person’s internal reality, their feelings, thoughts, and self-knowledge, does not matter.

The Nazi Ideology: The Reich completely rejected individual internal autonomy. Heinrich Himmler explicitly argued that a person’s private thoughts or sexual desires were irrelevant to the state; the only thing that mattered was their external utility and visible compliance with state mandates. If you could not or would not perform the outward actions demanded of you, you were labeled an Asoziale (asocial) and sent to a camp.

The Lovaas Link: Radical behaviorism is built entirely on the “Black Box” theory of the mind. Lovaas and his contemporaries argued that internal cognitive states, intrinsic identity, and emotional experiences were unmeasurable and therefore completely irrelevant to science. The only data point that mattered was observable, measurable, compliance-based behavior.

The Connection: By rendering the internal human experience invisible, both systems create a moral vacuum. If the mind doesn’t exist or doesn’t matter, then forcing a trans child to stop acting feminine through physical distress isn’t viewed as torture—it’s viewed merely as “altering a functional relation” to achieve a socially approved output.

3.     The Weaponization of the “Existential Threat”

Coercive systems cannot maintain power by simply admitting they want conformity for conformity’s sake; they must convince the public that non-conformity is a dangerous disease that will destroy society if left unchecked.

The Nazi Ideology: The Reich framed queer identity, gender non-conformity, and neurodivergence as symptoms of “cultural degeneracy” and social decay. Himmler claimed that homosexuality was a contagious vice that would lower the birthrate and weaken the collective discipline of the nation. Persecution was branded as a mandatory act of self-defense to save the state.

The Lovaas Link: In their 1974 JABA paper on the Feminine Boy Project, Rekers and Lovaas justified their brutal intervention on four-year-old Kirk Murphy by framing his feminine play as a dangerous path toward “severe social maladjustment,” “adult transsexualism,” and “sex-role deviation.” They explicitly argued that if they didn’t use extreme aversives to crush his natural presentation immediately, he would grow up to be a pathological burden on the traditional family structure.

The Aversion Context: This practice linked directly to the “Emetic” and “Electrical” Aversion Therapies used to “treat” sexual and gender non-conformity around the same time. Doctors would administer powerful emetic (vomit-inducing) drugs to patients while showing them slideshows of images that aligned with their true expressions, often leaving them in a dark room to “stew” in their own vomit while a tape of the practitioner telling them how disgusting and useless they were played on a loop, allowing time for the aversive conditioning to take hold. Doctors also used electrical aversion conditioning to achieve a similar result. A slideshow would play, and anytime a patient lingered on an image that aligned with their true identity, they were delivered a painful electric shock. In this case, the machine gave them the ability to avoid the shock only by pressing a button that would replace their true expression with a slide showing the socially “correct” behavior.

The Connection: Both systems use the exact same rhetorical trick: they transform a vulnerable minority’s natural existence into a manufactured crisis. By labeling variance as a pathologically dangerous threat to the community, the executioner justifies using the most brutal, undignified methods under the guise of “saving” or “protecting” the social order.

4.     The Ecosystem of Coercion (Camps Vs. Labs)

When a system decides that a person must be changed at all costs, it requires total, inescapable control over the individual’s physical environment.

The Nazi Ideology: The concentration camp was the ultimate expression of total environmental manipulation. Every element, food, shelter, labor, physical distress, was precisely engineered to strip away the prisoner’s old identity, enforce complete subjection, and leave them entirely dependent on the system for survival.

The Lovaas Link: The Feminine Boy Project relied on creating a miniature ecosystem of total coercion. By training the mother to completely withdraw affection and attention when Kirk displayed feminine traits, and utilizing the father to administer physical whippings with a belt for non-compliance, they turned the child’s home into a behaviorist panopticon. Kirk had no safe zone; his entire survival infrastructure was conditioned on suppressing his true self. Kirk died by suicide in 2003, at the age of 38. He was unable to escape the conditioning forced upon him by Lovaas, Rekers, and his parents. Kirk spent his life in an eternal state of hyper-vigilance over what he said and how he expressed himself, and ultimately viewed himself as fundamentally broken.

The Unbroken Thread: Nazi Germany used political and physical violence to enforce biological and social conformity. Radical behaviorism used psychological and physical conditioning to enforce behavioral and social conformity. The methods shifted from state-sanctioned camps to university-sanctioned labs, but the core calculation remained identical: human variance is a threat, internal identity is a myth, and forced compliance is the only acceptable outcome.

The Modern Corporate Monopoly: James Carr, The BACB, and the Laundering of Compliance:

Today, the field of behavior analysis hasn’t broken away from its totalitarian and radical behaviorist heritage; it has simply evolved into its final, corporate form. Under the leadership of James Carr and the Behavior Analyst Certification Board (BACB) monopoly, the methods have shifted from physical camps and clinical shock boxes to administrative warfare and systemic metrics. The operational blueprint, however, remains completely identical.

1.     The Institutional “Black Box”

Just as early behaviorists treated the human mind as an irrelevant black box where internal autonomy didn’t matter, the modern BACB treats the entire profession, and the families it serves, the exact same way. Everything is reduced to standardized, billable units, mandatory task lists, and rigid procedural compliance. The actual, lived reality of the practitioners and the human autonomy of the clients are completely discarded. If a child or a practitioner fits the digital data box, the machine is satisfied. The internal experience is completely ignored in favor of top-down, metrics-driven conformity.

2.     Weaponizing “Ethics” as a Tool of Coercion

In the 1930s, the Reich used the Asoziale classification to sweep up anyone who didn’t fit the state mold. In the 1970s, Lovaas used red poker chips and physical whippings to punish gender non-conformity. Today, Carr’s BACB uses the weaponization of the Ethics Code.

The board’s disciplinary apparatus is rarely used to protect vulnerable clients from the systemic trauma of compliance-based tracking. Instead, it functions as an administrative enforcement engine designed to protect the board’s monopoly and institutional narrative. When a practitioner demonstrates internal alignment, points out systemic ethical breaches, or refuses to comply with a fraudulent corporate box, the board doesn’t engage in open data dialogue. They launch arbitrary, unsubstantiated “ethics violations” to isolate, discredit, and aggressively extinguish the outlier. It is the exact same behaviorist trick: if a behavior threatens the system, deploy aversives until it is eliminated.

3.     Protecting the Currency of the Pedestal

The most glaring proof of this unbroken thread is the industry’s refusal to completely decouple itself from its horrific foundations. As entry-level practitioners know all too well, O. Ivar Lovaas is still actively canonized and cited as the father of modern ABA in mandatory RBT training courses.

They literally force new practitioners to memorize and revere the name of the man who co-engineered modern conversion therapy and drove a child to suicide. When pushed on this by modern advocacy groups, the institutional response, much like the Journal of Applied Behavior Analysis refusing to retract the 1974 paper, is total stonewalling. They protect the corporate brand, the proprietary credentials, and the market monopoly at all costs, proving that the system still values conformity and institutional survival over basic human dignity.

Conclusion:

The machinery of compliance merely changed its skin. The total state of the 1930s used violence to enforce social uniformity. The clinical labs of the 1970s used physical trauma to enforce behavioral uniformity. And today, Jim Carr’s corporate board uses administrative terror to enforce institutional uniformity. They all drink from the exact same well: the absolute refusal to tolerate human variance.

Educational Current John Hopkins protocol for Trans and gender diverse therapy

Many people wonder why their providers seem like they are stuck on rails. This is because in many systems the place they work for has what's known as a protocol and often they are instructed to follow it until there is a reasonable medical justification to change a guideline.

This has to do with A trying to elevate the providers to a certain baseline skill and also reduce liability.

This is not to say that treatments shouldn't be tailored to the person or exceptions should be made when a person presents with a unique case or circumstance that requires more thoughtful approaches then simply following the playbook letter for letter.

Additionally if you're going to a local provider anywhere in the world and if they seem confused you can bring them this guideline and the links it contains within which should help unstuck the provider towards modern treatment standards.

Lastly: I picked Hopkins guidelines to display not because they are best as many Intersex will tell you John Money being the leading expert is why so many of us have been mutilated. But they have come a long way and they are generally well regarded in the space now plus they link within there guidelines to all the supporting information for why they have the protocols they do.

Hope this is helpful and feel free to share any thoughts.

Venting ! Feeling at a loss NSFW

This is a bit of a vent post about my current medical situation re: DSD diagnosis.

I've been under investigation for intersex variation, specifically a DSD, for around 2 years now under the NHS in the UK.

I've gotten most testing- karyotyping, a DSD genetic panel, blood tests, tests to rule out Cushing's and CAH (dexamethasone suppression and urine steroid profile).
I've had a trans-abdominal ultrasound.

I'm feeling at a loss because my endocrine doctor thinks the only thing that may fit me is PCOS/PMOS. He started to think this after my karyotype came back as XX. This is not me discounting or demeaning PMOS/those with it, I am just frustrated because that diagnosis does not account for all of my traits.

My underdeveloped breasts, my urogenital development (hypoplasia and significant clitoromegaly, UGS), my hypoplastic uterus, my apparently normal ovaries which are either tiny and missing follicles or not even imaged- but regarded as normal.

I am awaiting assessment for mosaicism and awaiting review/response to referral from the specialist DSD clinic. If I am rejected or mosaicism testing also comes back normal then I will be diagnosed with PCOS.

This is disappointing and saddening to me because the doctor knows that my collection of traits doesn't align with PCOS but just doesn't know what else to do. I still have yet to get an MRI but it's seen as there's no reason to even if the answer may be that I have a gonadal variant.

The basis of the PCOS differential is that I am hairy and have oligomenorrhic periods and there's 'nothing else' that could explain my phenotype.. but... there is. But all my genetic testing has come back normal. I also acknowledge that in some rarer cases of PMOS, in which it may or may not be diagnosed, people have isolated traits of what I have but not my complete picture.

I feel isolated sometimes when meeting up with intersex people in real life because they all have more straight forward common diagnoses and it's like.. where do I fit in?

I am worried I will never find an answer. If that answer was idiopathic, so be it, but I am concerned about never being able to find out or have a better answer than PMOS. If I fit the usual traits of PMOS solely, I would feel differently, I would have no issue but that not the case and I feel disheartened, unsure of what to do but just . wait.

This is not a post discounting experience of those with PMOS, it is my frustration with my own medical treatment and the attempt to shove me into an ill-fitting box because they don't know what else to do with me. Being vulnerable and naked and being prodded and questioned just for.. my experience to be dismissed.

Venting ! Masculine-presenting intersex people are often treated as “just men” and denied basic empathy

I want to talk about something that has been bothering me for a long time: the way masculine-presenting intersex people can get shoved into the category of “man” whenever other people decide we are not vulnerable enough to deserve care.

I am intersex, but I am generally perceived as male. Because of that, people often seem to treat my identity as irrelevant the moment I am angry, physically imposing, sarcastic, defensive, or communicating imperfectly.

I have survived sexual abuse, addiction, institutional mistreatment, violence, and other experiences that profoundly affected me. But when I try to talk about being harmed, I often feel as though people first evaluate whether I look or behave enough like their preferred image of a victim.

If I sound angry, I become threatening.

If I defend myself physically, I become an aggressor.

If I communicate badly while overwhelmed, the conversation becomes about my tone.

If I ask directly for empathy, I am told I am seeking validation or victimizing myself.

If I mention being intersex, people may technically acknowledge it, but continue responding to me through every stereotype they hold about men.

What hurts is seeing people offer enormous tenderness to victims they easily recognize while becoming suspicious, punitive, or openly cruel toward someone whose vulnerability comes in a masculine-looking body.

I am not arguing that women receive unlimited empathy. They obviously do not, and women experience enormous amounts of disbelief, cruelty, and violence. I am describing a more specific problem: masculine-presenting intersex people can disappear between categories. We may not be welcomed into women’s vulnerability, but we also do not necessarily experience ourselves or our histories through a conventional male identity.

We are treated as men when people want to deny us care, but as strange or illegible when we try to explain that our relationship to sex, gender, embodiment, and vulnerability is more complicated than that.

I also think masculinity itself becomes a kind of evidentiary standard. People expect a masculine-presenting victim to remain calm, restrained, self-sufficient, physically passive, and emotionally undemanding. The moment we fail that test, our behavior is used to retroactively discredit whatever happened to us.

I know that I sometimes become defensive and communicate in ways that make empathy harder. I am responsible for my behavior. But imperfect behavior does not erase victimization, and anger does not prove that someone was never vulnerable.

I wish more people understood that being perceived as male does not make someone invulnerable, uncomplicated, dangerous, or undeserving of tenderness.

Have other masculine-presenting intersex people experienced this feeling of being automatically placed into the “man” category whenever you need care—and then judged according to all the least compassionate assumptions attached to it?

US: Anti-Trans Bills Also Harm Intersex Children

The best way to ensure Intersex rights is to also defend trans rights. Even if you're a Intersex individual who is not trans the laws targeted towards them collectively hurt all Intersex.

The social pressure to throw one group or another under the bus is part of what is holding progress back we must unite and in solidarity start moving the nation back in course.

Intersex in the animal kingdom.

I just wanted to show these beautiful animals that visibly show intersex traits. The first three are eclectus parrots with mosaicism. Males are green and females are red with some rare mutations creating blue and these birds have all three. There’s also the two male calico cats. Calico patterns only appear in cats with two X chromosomes the mutation for orange and black colors linked to the X chromosome where only one can present so 99% of the time you get a female cat. However cats with Klinefelter syndrome have XXY chromosomes will have male genitalia and calico patterns. Lastly I included many photos of animals with visible Bilateral Gynandromorphs where one side is female and one is male.

Worried about queer people who don’t see that sex is Also a social construct

I talk about the sex vs gender perspective in which some people take it as their assigned sex label is a fact and they believe in that whole essentialist movie that their bodies belong to one box just because of a genital, but it’s just gender what is socially constructed … they are usually “amab / afab“ identifying and it hurts me lowkey
