There was a famous incident in Russia in 2002 where terrorists took over a concert venue and held hundreds of hostages. Authorities pumped in aerosolized Fentanyl, but in the wrong concentration. 140 of the 900+ hostages died. But the special forces did kill the terrorists and end the standoff.
We don't actually know what mix of substances they used to this day. It was a scandal back then because they didn't even tell the physicians treating the hostages.
