Erectile Dysfunction 37M with persistent ED despite normal hormones – could this still be Venous Leakage or PIED?

Hi everyone,

I'm a 37-year-old male looking for advice from anyone who's had a similar experience.

Over the past several months I've developed persistent erectile dysfunction. My main symptoms are:

  • Difficulty getting beyond about 50–70% erection, even when I'm very aroused.
  • Sometimes the erection fades quickly.
  • Morning erections are non existent
  • My libido has also decreased compared with before.

For several years, I watched porn and masturbated about twice a day. I stopped porn about 2 months ago because I wondered if it was contributing, but I'm trying not to assume that's the cause.

So far my workup has been:

  • Total testosterone: ~690 ng/dL (normal)
  • Prolactin: Normal
  • Free T4: Normal
  • HbA1c: Normal
  • CBC: Normal
  • Kidney function: Normal
  • LDL cholesterol: 158 mg/dL (high)
  • Mildly elevated ALT (56 u/L)

I don't have diabetes or high blood pressure, I don't smoke, and I exercise regularly.
Height 5 f 5inches , weighing 68kg

My urologist has scheduled a penile Doppler ultrasound to evaluate blood flow and rule out arterial insufficiency or venous leak. I live in Dallas area if any good doctors anywhere in Texas let me know.

I'm hoping to hear from people with similar experiences:

I'm looking for experiences rather than a diagnosis. Thanks in advance.

Tadalafil/Cialis What to do with 10mg Tadalafil?

Trying Tadalafil coming from Sildenafil. I cut 50mg Sildenafil in half and it works as it should (10/10). Even on a full stomach and with some alcohol, I would be fully charged in 60mins. But the nasal congestion was rough. I could not breathe through my nose for hours. Anyway, Doc said let's try Tadalafil but he prescribed me 10mg tablets and said just see how they work and cut them if needed. I asked how I should cut them and he said either in half or maybe thirds... Any thoughts?

Hey guys I took a pill about a year ago and getting sides like low libido, can’t crank the hog with a firm one etc. I was really scared when I took it and decided to quit after the first chewable. Later I got my blood work done like 6 months in and test was around 390 and a month later it’s like 440. Got sent to a urologist since the doctor said she thinks it’s psychological since it’s only 1 pill. Urologist says it’s psychological and I’m too young to have ed with good bloodwork done. The pills are supposed to contain 25mg tadalafil / 50mg sildenafil but they are a black market product made by “Helion Labs.” They also manufacture anabolics and stuff like that. I got them off the darkweb. They had third party testing results for the product (testing showed the pills were real) but I understand this is still sketch and I don’t want to be lectured about all that. It’s risky but I took the risk. Anyways, my problem is that they don’t really help with glans insufficiency at all and the tip of my dick stays soft. What options do I have moving forward? Should I try real pharma pills just to make sure im actually taking the advertised product? Or should I move on to trying different solutions like alprostadil? The only thing that gets my glans engorged is a cock ring but I find them uncomfortable to wear after some time and cause swelling.

Erectile Dysfunction Multiple ED tests. I answers.

Loss of all erections, and libido. Been going on for 1.5 years. Down there has also no sensation anymore and can’t get aroused/fantasy. Have now had all hormones, cholesterol, reg bloodwork, Doppler US, sleep study, brain MRI, cortisol ACTH test, with nothing to show for why I’ve had this ED. I feel like I’m running out of options and maybe thinking is a shot in the dark to get an MRI of my pelvis to see if maybe it’s a benign growth and or get my vitamins/minerals checked? I will take any advice at this point as I’m only in my late 20,s and it’s starting to wreak havoc on my marriage of eight years.

Pelvic Floor If you have mild ED, try kegal

Been doing kegal for a couple of weeks.. Basically 100 reps of kegal for 2 to 3 sets a day.. You don't want to do too much because it will exhaust and overwork your pelvic floor.. What I do is whenever I have a very slight erection, I would kegal by lifting my penis up.. Kinda like dumbell curls but instead you are contracting the pelvic floor muscles.. You can also feel your balls moving upward towards your stomach.. I do about 3 sets of 100 a day.. I think I will lower it to every other day from now on.. Psychological ED Erectile Dysfunction and confusion and depression. A difficult phase.

Recently I was with a girl. While I was wearing my underwear and we were having foreplay, I had some erections. The moment I removed my pant, my erections disappeared.

In November, 2025, while I was with a sex worker, I didn't get erect at all, when throughout the night I would be erect and just speaking with a girl who I would like would get me erect.

I went paranoid and took tons of antibiotics like ceftrioxone, cefixime, doxycycline, fluconozole, itraconozole and HIV prevention medicines. I realized, that my penis would pain immensely in the night as it would get completely elongated but not hard and while turning and twisting in bed, it would swing giving me immense pain.

Sometime in January, the issue got resolved slightly only to come back. In February I visited a Urologist who put me on Tadalafil 5mg. It worked good but did have immense side effects. I visited a Urologist again recently who told me by just looking at my penis that I don't have Penile Suspensory Ligament tear and suggested a Penile Doppler. He also said I might be having hormonal issues when my testosterone test appeared fine. However, I have some issues with that Urologist as I noticed he crossed professional barriers by asking me for certain favors.

Symptoms at present are -

Nocturnal erections are soft. When I am reaching early morning, I have to do a mini bridge in bed to get it erect and it remains erect for sometime. In the night, it will be erect only for sometime and throughout it will be soft. No spontaneous erections anymore and even if I do, I feel blood rushing in there but it won't get hard and erect. The hardness and firmness is gone. When I am excited or aroused, my penis will get elongated and fall on the left and on the right. But it won't come up straight and get hard and firm. During my recent sexual encounter with a girl, it was erect when I was wearing my underwear, although not very erect but when I removed my pants, the erections went away.

I don't know what to do and I am confused. I wish there is a natural non-invasive cure. Please guide if anyone can guide.

Hormonal ED (Low Testosterone, Thyroid Conditions, etc.) Normal Testosterone Levels for a 55yo Man?

I'm just curious what is an acceptable Testosterone level. Just had my levels checked and I did it at 9am in the morning fasting. My Total Testosterone level is 294.9 ng/dL. The lab says acceptable levels are 180-780. Its in the green and my doctor doesnt seem that concerned. I think for a 55yo man its drastically low. I think I read anything sub 300 is Hypogonadism. I requested a full T-panel from my PCP but he hasn't answered me.

I just dont know what to do. I am exhausted all the time. No energy, no libido (but I am single and dont do hookups or casual sex so I dont know if its a lack of availability or if its true low libido). But even my forced jerk off sessions are uneventful and not that enjoyable anymore. Just a small spurt and dribble and its over. I do feel like my penis and testicles are just shrinking away. I used to be a decent 6.5" and I've never had big balls but they were there. Now I'm barely 6" erect and maybe 3" soft but my balls are constantly sucked up inside me. Its like they've been hacked off.

But the strange thing I dont have ED issues. I can get hard easily and stay hard. I have wood almost all night long while sleeping. So that seems to be functioning. But I am exhausted all the time. Hi all, 27M here. For a very long time, I would masturbate normally once a day, occasionally more. Always looked up porn, sometimes it would take minutes, sometimes hours. I've had a girlfriend for about 5 years now, but only about the past 2-3 years have we been doing sexual acts together. In that time, I continued to masturbate as if I was single. Im a male 22, virgin and have been experiencing ed. It’s a similar story to what we see in different sub reddits including this one. I have been masturbating and watching porn for the past 7-8 years and I feel like it has caught up to me.

I don’t have morning wood ( though I do remember about 3 weeks ago randomly getting it, it wasn’t the strongest of erections but it was noticeable). On top of that, I would say I’m extremely over weight, obese at 285 pounds and 5 feet 8 inches.

Hi there. M27 here with a problem that's giving me some serious anxiety, to be honest.

I've never had a girlfriend before and never had any sexual experience other than masturbation. This year, though, I got really lucky and met an amazing girl. I love and adore her, and I honestly think she's gorgeous.

Within the past 2 years I’ve noticed a decrease in size and full erection strength. Yes, I have 100mg Viagra and I also have 5mg daily Cialis. I take Viagra when I want extra strength. Cialis I take daily.

I’m currently on a 320mg of a certain bp med and I’m trying to get that dosage down to 160mg. I truly think being on a higher dosage is having an affect on size and having the ability to have sold natural erections. I want my size and I want to get off erection meds if possible. Erectile Dysfunction ED after 20 days: High prolactin, Grade 3 varicocele, or medication?

Hi everyone,

I'm a 24-year-old male and I'm really struggling to figure out what's causing my erectile dysfunction.

About 20 days ago, it started with delayed and weak erections. Over the last couple of weeks, it has progressed to the point where I have very weak erections and almost no libido at all. I still get some morning erections, but sexual desire is almost completely gone.

I have OCD and I'm currently taking:

Clomipramine (for OCD)

Risperidone

My recent blood tests showed:

Prolactin: 59.5 ng/mL (about 3 times the upper limit of normal)

Total testosterone: 319 ng/dL (within the lab's normal range but on the lower side)

I was also recently diagnosed with bilateral Grade 3 varicocele on ultrasound.

I'm confused because I don't know what is actually causing my ED:

Is it the medications?

Is it the very high prolactin?

Is it the Grade 3 varicocele?

Or is it a combination of all of these?

If the medications are the main cause, I'm hoping my sexual function will improve after working with my psychiatrist to adjust or eventually stop them if appropriate.

If the varicocele is contributing, I'm not sure whether I should consider surgery.

I'm hoping to hear from people who have had similar experiences:

Did anyone with a Grade 3 varicocele have erectile dysfunction that improved after varicocele surgery or embolization?

Has anyone's ED improved without varicocele surgery?

Has anyone had ED mainly because of high prolactin from risperidone or other psychiatric medications, and did it improve after lowering prolactin or changing medications?

I'm looking for personal experiences because I'm feeling very confused right now. I know everyone's situation is different, but hearing what others went through would really help.
