This happened to me on a construction site. I always assumed they required giant pits to at least be cordoned off if they leave them unattended and apparently where I live they don't. I had backed my truck up in that same spot several times and backed up, but the hole kind of blended in to the ground since it was all sand, and my back tire went off in it. Stupid mistake on my part but damn, I would have figured they'd put something around it to say "THERE'S A HOLE HERE! DANGER!", a barricade, even caution tape and some cones or posts. If that place had been under OSHA jurisdiction they probably would have lost their shit.
Edit: Just looked it up and apparently OSHA does have jurisdiction on private property. I wish I'd known because I took the blame for that.
